RECORD OF PROCEEDINGS AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS IN THE MATTER OF: DOCKET NUMBER: BC-2009-00635 COUNSEL: NONE HEARING DESIRED: NO _________________________________________________________________ APPLICANT REQUESTS THAT: The Air Force Achievement Medal (AFAM) he was awarded on 26 March 1987 be added to his DD Form 214, Certificate of Release or Discharge from Active Duty. _________________________________________________________________ APPLICANT CONTENDS THAT: He was awarded the AFAM on 26 March 1987; however, it was never added to his DD Form 214. In support of his request, the applicant provided a copy of the certificate awarding the AFAM and a copy of his DD Form 214. The applicant’s complete submission, with attachments, is at Exhibit A. _________________________________________________________________ STATEMENT OF FACTS: The applicant enlisted in the Regular Air Force on 19 August 1983. He served as a Metals Processing Specialist. The applicant was honorably discharged on 30 April 1987. He served 3 years, 8 months and 12 days on active duty. _________________________________________________________________ AIR FORCE EVALUATION: AFPC/DPSIDR recommends denial. DPSIDR states without the special order authenticating the AFAM, they cannot verify entitlement to the AFAM. The DPSIDR complete evaluation is at Exhibit B. _________________________________________________________________ APPLICANT'S REVIEW OF AIR FORCE EVALUATION: On 1 June 2009, a copy of the Air Force evaluation was forwarded to the applicant for review and comment within 30 days (Exhibit C). As of this date, this office has received no response. _________________________________________________________________ THE BOARD CONCLUDES THAT: 1. The applicant has exhausted all remedies provided by existing law or regulations. 2. The application was not timely filed; however, it is in the interest of justice to excuse the failure to timely file. 3. Sufficient relevant evidence has been presented to demonstrate the existence of an error or injustice. Although the applicant was unable to provide a copy of a special order awarding him the AFAM, the majority of the Board believe the documentation he provided in the form of an AFAM citation provides a reasonable basis to conclude he was awarded the AFAM. In the interest of equity and injustice, the majority of the Board elects to resolve any doubt in favor of the applicant and recommends his record be corrected as indicated below. _________________________________________________________________ THE BOARD RECOMMENDS THAT: The pertinent military records of the Department of the Air Force relating to APPLICANT, be corrected to show that his DD Form 214, Certificate of Release or Discharge from Active Duty, be amended in block 13, to include the award of the Air Force Achievement Medal. _________________________________________________________________ The following members of the Board considered AFBCMR Docket Number BC-2009-00635 in Executive Session on 21 July 2009, under the provisions of AFI 36-2603: , Panel Chair , Member , Member By a majority vote, the Board recommended to correct the records, as recommended.
